Job Summary:

Case management involves working with the individual and others who are identified by the individual such as a family member or guardian in developing an individual support plan and services. Case management works closely with the individual to assure higher on-going satisfaction with the process and outcomes of the supports, services, and available resources.

1. 30% Complete face to face and related contacts to make arrangements for activities which assure that: a) desires and needs of the individual are determined b) the supports and services desired and needed by the individual are identified and implemented c) housing and employment issues are addressed d) social networks are developed e) appointments and meetings are scheduled f) a person-centered approach to planning is provided g) informal and community supports are utilized h) the quality of the supports and services as well as the health and safety of the individual are monitored i) income and benefits are maximized based on need j) activities are documented k) plans of support and services are reviewed at such intervals as are indicated during planning (at least annually)

2. 15% Coordinate, plan, and monitor case management process.

3. 5% Complete evaluations and assessments at least annually

4. 5% Assure freedom of choice to providers.

5. 20% Develop Individual Support Plans based on the Individual's self-assessment.

6. 5% Work with others on the team to assist the individual with access to community resources.

7. 5% Provide on-call services as assigned.

8. 5% Provide Quality Assurance Services (ie: attend meetings on a regular basis and review records on a regular basis).

9. 5% Assist in training or in-service of staff.

10. 5% Provide information to support staff to aid with providing needed service to individual


Minimum Qualifications:

  • Bachelor's Degree
  • 1 year experience working with individuals with intellectual and/or developmental disabilities
