JOB SUMMARY:
The Care Manager works under the supervision of the Program Manager and Clinical Director/Supervisor. This position will focus on provision of person-centered services and comprehensive care management including planning and coordination of all physical, behavioral and support services to increase independent living skills and positive community involvement for enrolled participants.
